Tractor-trailer crosses center line in Mendon crash

MENDON — A two-vehicle crash on U.S. Route 4 near Craigs Lane sent one driver to the hospital with significant injuries early Tuesday morning.

Vermont State Police troopers from the Rutland Barracks responded to the scene at approximately 5:11 a.m. on May 20.

Forrest Adams, 39, of Hartland, was driving a 2026 Freightliner Cascadia westbound toward Rutland when he crossed the center line, according to police investigation.

The tractor-trailer struck the side of an eastbound 1998 Ford F-150 driven by Alexander Hernandez-Reyes, 47, of Castleton.

Hernandez-Reyes suffered significant injuries in the collision and was transported to Rutland Regional Medical Center.

Adams was wearing a seat belt and reported no injuries.

The Freightliner sustained significant damage, while the Ford F-150 was totaled in the crash.

Road conditions were dry and weather was clear at the time of the incident.

Rutland City Fire Department and Regional Ambulance Service assisted state police at the scene.

Civil violation charges are pending against Adams, according to authorities.
