HAMMOND, NY — New York State Police responded to a two-car personal-injury motor vehicle collision on State Highway 12 on Dec. 1 at approximately 6:17 p.m.

Upon arrival, troopers observed a 2022 Ford and a 2017 Mazda positioned in the roadway.

Troopers interviewed the operator of the Ford, identified as Christopher P. Reynolds, 37, of Ogdensburg.

Reynolds stated he was traveling north on State Highway 12 when he looked down and failed to observe the 2017 Mazda slowing to make a left-hand turn, subsequently striking the Mazda in the rear.

Evans, who is currently pregnant, reported experiencing back pain.

Morristown Rescue arrived on scene and evaluated Evans.

She was transported to Claxton-Hepburn Medical Center for further treatment.
